Medicare Part D prescription drug plans are regulated but not standardized. Each plan is different in the drugs it covers, its formulary, and how much you’ll pay. However, all plans must meet a minimum benefit standard—covering at least two drugs in every category plus nearly all drugs in six protected categories:
Immunosuppressants
Antidepressants
Antipsychotics
Anticonvulsants
Antiretrovirals
Antineoplastics
Understanding Medicare Part D Formularies and Tiers
Every plan has a formulary (drug list), usually divided into cost tiers:
Tier 1: Common generics, often no copay.
Tier 2: Some generics and common brand names, usually low copay.
Tier 3: Preferred brand-name drugs, higher copay.
Tier 4 & 5: Specialty drugs, cost-sharing via coinsurance.
Formularies can change each year, so it’s important to review your plan during Annual Enrollment.
The Importance of Network Pharmacies
Most plans offer cost savings through preferred network pharmacies. These pharmacies—whether local, national chains, or mail order—have lower negotiated prices. Using in-network providers is one of the best ways to keep prescription costs down.
